ALARA Exposure Report
This report is used by the Radiation Safety Officer or account
authorized representative to evaluate the dose segregation for monitored
individuals who exceed default Level I or Level II quarter to date or
year to date dose thresholds, or facility specified threshold values.
The report also provides the percentage of the regulatory limit the
quarter to date or year to date dose values represent. This report only
includes individuals who exceed the threshold dose values.

ALARA Letter
The ALARA letter details the same information as the ALARA Exposure
Report but in a printed letter format. The RSO can provide this letter
to the wearer.

GDS Dose Management Report
The GDS Dose Management Report consists of three separate reports:
- The Individual Summary Report provides information
on the number of dosimeters returned for processing for each wearer by
dose type. Results are summarized per wear period.
- The Location Summary provides the same information
as the individual report, but also provides data on the average dose
per dosimeter reported by type for each specific wear period.
- The Account or Facility Summary Report provides
information on the number of dosimeters returned for processing for
each wearer and reported for each dose type. Results cover per month,
quarter and year, for the entire account, average dose per dosimeter
reported, and various dose type reported for each specific wear
period.

Dose Adjustment Confirmation Report
This confirmation report provides documentation verifying a change in
dose history since the last Occupational Radiation Exposure Report was
provided to the facility. Upon receipt of a written request by the
Radiation Safety Officer or account authorized representative, an
adjustment to the dose history is noted to reflect the requested change.
A confirmation report letter is then provided indicating the reason for
the dose adjustment, i.e., a dose has been estimated, lifetime history
added, dose transfer or change of dose for a specific wear period for an
individual.

Statistical Summary Report
The Statistical Summary Report provides a breakdown by dose levels (deep
dose reported in Rem) the number of dosimeter reported doses within each
level. The report can be prepared to reflect a single location or all
locations. If a person wears two dosimeters, each dosimeter's cumulative
dose will be categorized into one of the dose levels. Therefore, this
report does not provide the number of individuals in each level, rather
the number of dosimeters reported.

Unreturned Badge Report
This report provides a detailed listing of those badges not returned and
not scanned back into the GDS system. This report is useful for clients
to determine which badges have not been returned at the end of a wear
period. The report can be accessed by an on-line registered client for
quick review and printing.

Preliminary Radiation Exposure Report
For emergency situations, this preliminary report can be provided for
informational purposes only to a client prior to receiving the official
Occupation Radiation Exposure Report.

History Detail Report
The History Detail Report shows the dose history detail by wearer, badge
type, and wear date, and process number. The report will normally be
generated as a spreadsheet and can be used to quickly review the
detailed processing history of an individual.

Environmental Monitoring Report
This report provides a complete listing of locations and reported
exposures. A control value is usually not subtracted. The Environmental
Report can be customized to meet the reporting needs of the facility,
including exposure per standard month or quarter. This information is
helpful when comparing data from other agencies and across several
years.

Sealed Radioactive Source Leak Test Certificate
This report details the isotope, the amount of radioactivity, source
manufacture and description, who conducted the leak test, who analyzed
the source and final results. The source is considered to be leaking if
0.005uCi (185 Bq) of removable radioactive material is found.

Control Element Value Report
This report lists the value of the element responses of the control
badges returned by the facility and assists the reviewer with
information for investigations.

Neutron Radiation Exposure Report Letter
This is the cover letter for the Neutron Report and explains that the
neutron dose is reported in units of mrem, when the client uses only the
GDS Badge Type 9 (CR39 only).

Neutron Radiation Exposure Report
This report provides the Neutron Dose Equivalent in mrem a specific wear
date for users of the GDS Badge type 9 (CR39 only).

Total Dose Report
The Total Dose Report is a report that provides the total dose received
by an individual during a time specific period from all accounts or
locations monitored for the individual by GDS. The report may also
provide lifetime data if that information is to be provided to the
current service provider. Due to state and federal Privacy Act
requirements, the facility's authorized representative and the
individual monitored are required to sign a waiver form in order for the
individual's dose history to be included in the Total Dose Report.

Dosimeter High Call List
This is a courtesy notification faxed to customers to inform the
institution that one or more of the wearers have exceeded the specific
notification level of the institution. This is in addition to the
Occupational Radiation Exposure Report (Dose Report) that is mailed to
the institution.

REIRS Report
Radiation Exposure Information and Reporting Systems (REIRS) is a
database which contains occupational radiation exposure records as
required from the licensees. The format is very specialized and the data
is sent to the client electronically or on a disk. The client then adds
additional information that is pertinent to the license. The provided
report follows the protocol stated in regulatory Guide 8.7.

HP Assist Information Disks
HP Assist Disks contain specialized data information in a format that is
compatible with the HP Assist Program used by some facilities. HP Assist
is a database of dose history and the disks are normally prepared at the
end of each quarter. The data disk contains dose history on the wearer,
the particular badge worn by the wearer, and a summary record providing
data over a certain period of time. The combination of all these records
provides the basis for the client to prepare their own NRC Form 5.
